People who are afflicted with chronic illnesses like hypertension, diabetes, heart problems or people who are regular visitors of a hospital will have a higher risk of being in an emergency situation where every precious second counts. 
In order to save lives and prevent needless disabilities or deaths in emergencies like a heart attack or stroke, it is advisable that hospitals have an information system that can retrieve patient information, specifically the medical record including current medications, past health history, diagnostic exams and the like at an instant.  Let's just call it an MID, a medical identification device.  As soon as the ambulance arrives in the patient's location, the medical staff will only have to scan the MID and everything needed to help the patient will be visible in front of the ambulance doctor and in the hospital.  This way, the medical doctor can readily institute first aid and stabilize patient while in transit without wasting any additional time to extract the health history of the patient. 
On the other side, the emergency room physician, while waiting for the patient to reach the hospital, can prepare what will be needed and call the specialists who will be attending to the patient. This will save precious seconds that may cost a life and/or prevent disabilities.  The emergency room physician will also have access to what is happening real time in the ambulance because the latter is hooked to the hospital system.  Assessment of the patient will be real time and the patient will have access to specialist's opinion even if the patient is still in transit.4
